Dica rápida sobre a dose de apixaban em pacientes com fibrilação atrial:

   
• de forma geral ⇒ 5 mg via oral de 12/12h
   
• usar 2,5 mg via oral de 12/12h se o paciente tiver ≥ 2 dos seguintes fatores: Cr≥1,5 mg/dL, idade ≥80 anos, peso ≤60 kg
